One star because it''s the only way to leave a review this rehabilitation is terrible my grandma mother was here for 3 weeks and developed DTI ( deep tissue injury ) on both of her ankles… I’m a CNA so I know exactly how this happened her legs was kept elevated among other things the worked attempt to be rough with her while providing care… imagine if this was you or your loved one how would you feel… if your love one has to go to rehab choose another rehabilitation facility because this definitely isn’t a good one The pictures are too graphic to upload Management- owner Don’t not reply to my review saying how sorry you are that I had negative experience Fix the problem after reading other reviews this place definitely need some reinforcement

This facility has some serious problems in dealing with the care of my husband as a short term patient. Here is a list of problems that I experienced - 1. Facility failed to educate or inform patients or relatives of patient of the operations. Not knowledgeable of doctors on board nor changes in medical orders. Just assumed that once patient when to his private physician that orders were being followed. 2. Food was cold due to staff taking their own time in delivery to patients. 3. Having to come in and always ask for a broom to clean patient’s room. Watch staff cleaning rooms - asked on occasions if they would move nightstands to get all of the trash under them. 4. Failure to respond to call lights. 5. Difficulty in getting responses from nurse care staff on treatment of wounds. 6. When asking for assistance with love ones - staff always promised to come back - did not do so unless I ran them down screaming for help. 7. Staff ignoring relatives when asking questions about love ones. 8. Director was quick to answer complaints in favor of staff instead of during research to validate the complaint. 9. Staff had no regards to ensuring that private conversations were not heard or discussed in hallways. 10. Cleaned clothing was provided by relatives for the patient - however, found the patient wearing soiled clothing two days in a roll. 11. Staff would stand in lobby and just watch me ring the doorbell for entry. 12. Staff would park in visitor parking slots instead of parking in rear of building. 13. Husband being a dietetic - would find the cotton swap with blood on it from checking blood levels on the floor instead of trash cans. 14. When asking for assistance - staff was quick to tell you that it was not their job responsibility nor were they eagle to help locate assistance. 15. Has conversations with senior staff to address concerns. Only assurance given that my complaints would be discussed with staff. 16. Had to come in everyday to ask if clean dressing has been applied to womb. No marking was placed on dressing to indicate that dressing had been changed. 17. Non professionalism was demonstrated when asked certain questions in regards to care of patient - especially from the nursing staff. 18. Failure to educate all concern parties or individuals on operations of center was a crucial and critical factor for me. Unaware of the practices of the center made it difficult to communicate or address concerns.

You can tell that Patient Care is a priority from Admission to Discharge. Jasmin and Shermaine are great. I never have to request additional information, because they are both very precise. I appreciate the professionalism and the willingness to go the extra mile to make families feel at ease. Communication is important. I have received positive feedback from families regarding the Facility and the two Social Workers mentioned above. Thank you!

BEWARE, I repeat BEWARE!! Stay far away from this facility and do not entrust them with your loved one unless you have exhausted all other options. If that is the case keep a note pad and pen with you as well as a fully charged mobile to take pictures at all times and document EVERYTHING because you might need it later! I''m giving one star because the initial meeting and tour of the facility went well and the physical therapy team is courteous and professional . Everything else went downhill from there. My loved one was sent there for rehab but only got worst in their care. I visited almost EVERYDAY and at different times on purpose. Often times she was wet and/or in the same exact clothing. I was told that she would get a bath twice a week unless I requested otherwise. I began to take notes of EVERYTHING I saw and took pictures to support my findings. I also asked that my notes be placed in her medical file (after I took a picture of it with my phone of course). The bathroom in the room was deplorable so I took pictures and reported it. See attached. Meals were left in rooms even if the patient was at a dialysis clinic and meds are all issued to everyone on the same schedule even if your schedule says otherwise. Not once when I visited were required GERD meds administered 30 minutes prior to breakfast/dinner even though I mentioned it. When I requested that she be dressed everyday and receive assistance with brushing her teeth and other general hygiene, I was told that I had unrealistic expectations. The final straw was the ER visit and a sepsis diagnosis ending with a stay on the floor just above/below ICU. That was negligence at it''s finest! I could go on and on and I''m sure someone from the facility or corporate will reply to say they are sorry WE had such a bad experience and that they are working to make things better. DON''T BELIEVE A WORD OF IT!

DO NOT SEND YOUR LOVED ONES HERE!!!!!!!! If I could rate a negative number, I would. My mom was transferred to this facility from the recommendation of social worker at Emory John''s Creek on Thursday 7/28. I had met with Admission Person Lisa Synder the day before. She talked a good game and assured me that my mom would be in good hands with the highly skilled nurses on staff. WHAT A LIE!!!! In all my past experiences interacting with nurses. I''ve never encountered such unprofessional, rude, incompetent Healthcare staff. The doctor never responded to any of my inquiries and today my mom was ambulanced to Northside from internal bleeding and had to have blood transfusion and emergency procedure done. I had texted Lisa and like everyone else at this pathetic facility, she never responded. I could go on and on of the negligence that my husband and I observed ourselves with the care (or lack of) of my mom. Wanted to quickly post to warn people to not send your loved ones here!!!! I have documentation of all the staff that were negligent resulting to the status of mom tonight. As many people are telling me now as I research my options, they wouldn''t even leave their dogs there.... So I got the generic response that EVERY Negative reviewer received - see below, yet when I call the number listed, it''s an automated voice message with only 3 prompt options not even related to Customer Care. What is wrong with this Provider????
