Here at Lone Oak we offer three different floor plans for studio apartments, as well as three different floor plans for our one-bedroom apartments. Each apartment is equipped with walk-in showers, beautiful cabinetry, and granite counter tops. Each apartment also has a pendant call system that can be utilized anywhere on Lone Oak’s campus and a pull-chord system, in which our residents can use at their convenience when needs arise outside of scheduled services. With that, we also have round-the-clock staffing including, Med Staff, Care Staff, and a full-time RN who is also on-call 24/7.
We have an ever-changing Life Enrichment Calendar and at your family’s convenience, they can join us on tours of local wineries, museums, parks, festivals, as well as indoor events like lecture series, cooking classes, liqueur making, arts & crafts, and more. It is our commitment that our residents get not only excellent care within our community, but also a life that’s engaging and inspiring.
We offer 5 Levels of Care here at Lone Oak so as needs rise – we’re here every step of the way. Our Care Plans are specific to each of our resident’s needs and each level of care is only $400, which is quite affordable compared to other options. At Lone Oak there are no hidden fees. You won’t see any RN fees, medication management fees, or a la carte “bundled” fees – which means our residents are only paying for the services they need, so there won’t ever be any surprises on their month to month statements.
We have a Medicaid contract here at Lone Oak. We find it fair that if, at any point, any of our residents deplete their own person funds, or savings, they can utilize Medicaid and SDS assistance to stay here at home. Our residents won’t ever see a change in services, experience, enrichment, or receive a move-out notice due to depleting their own private funds.
There are two hospitals within 25 miles of Lone Oak Assisted Living, Sacred Heart University District which is 4.2 miles away, and Mckenzie-Willamette Medical Center, which is 6.7 miles away.

They don''t pay there workers
It looks nice from the outside. Never gone inside as I pass it to go visit at McKenzie Living; which is a group of 4 Large houses, each with 15 private rooms. It is a specialized group home for folks with traumatic brain injuries.
I worked here and don’t recommend. I still haven’t been paid and I have been waiting since the beginning of December
Horrible place to work for. The caregiver ignore the residence and don’t care if there in pain. The have sketchy people working for them, that steal from you and threaten you. They don’t pay you right. And won’t give you the money they owe you. Then when you call a lawyer they lie to them to. ...

The leave residence wet and dirty for hours. The boss knows what’s going on and dosnt care. There is no commutation and no one tells the truth. I worked here for 5 weeks making 17. 50 a hour and they only paid me for 3 . And the whole time I was there I sawl residents not being taken care of. Bosses that dong care what’s going on. Caregivers who neglect the residents. And I still haven’t been paid right!!! Oh and don’t mention all the threats

I do not recommend this facility. Frontier Management has fired/replaced all but one of the former employees. The residents with personal funds just got a second rent increase this year-$1600 total. My mom pays over $8,000/mo and is eating sandwiches for dinner. The sandwiches consist of a slice of meat or cheese and 2 pieces of bread. They run out of food too and have to reduce the portions and serve on salad plates. No one will answer my questions, they just say it's getting better but it's clearly not. Good luck if your loved one has special nutritional needs--like being diabetic. The hourly staff are doing the best they can and are caring so I gave 3 stars for them. I recommend to avoid this place.
