Understaffed. Not notifying family of med changes, behavioral issues. Neglecting care by not properly feeding or doing routine checks . Patient ripped out feeding tube and was left overnight with stomach acid all over him resulting in 2nd degree burns covering one side of his body. Patients not being exercised as needed… in two years time patient went from being able to sit up by himself to being unable to straighten himself out due to muscle deterioration from improper exercise. Multiple attempts to communicate with the VP have been ignored for going on two weeks now. Full medical records were requested and only part of them received. The records that were received were a total mess and most of them not even dated. Place is a mess.
