PLEASE DO NOT GO TO WELLSPRINGS for skilled nursing. Avoid it at all costs! My grandmother was recently sent there after weakness from an illness. When she got there she was not on oxygen, was eating, and was able to walk. She was there for approximately 8 days before we switched her to another facility due to rapid health decline and negligence from the staff. In those 8 days with Wellsprings the following events happened: ⁃ She fell two times due to facility not using gait belt with transfers. Note: Family was not notified of fall! ⁃ Not seen by the in house doctor until day 5. ⁃ Told us she was receiving all her medication to only find out a week later it had just came in and she was never given it. ⁃ Given orders for a stat x-ray on day 6 that was never completed! ⁃ Overall lack of communication between staff and staff and family members was lacking. When my grandmother was finally transferred, she was too weak to transfer independently and on put on emergency oxygen. The new facility promptly got chest x-ray and she was diagnosed with pneumonia the SAME day. They also checked her vitals/blood test which showed she was at critical levels and rushed her to the hospital where she died 3 days later. I truly believe the neglect from this facility ultimately led to my grandmother’s death. Please NEVER send your loved one here unless you are ready to stay with them 100% of the time and are ready to hold Wellsprings accountable. I only wish we would have transferred her out sooner as we believe the outcome would have been different.

My parent spent approximately a year in the facility. We paid of our pockets because we hoped she would not just receive physical care but actual “ memory care” as they professed to offer. ( activities to help inhibit loneliness, provide comfort etc). I visited twice a week at random times of the day and I saw them provide an activity only once. She languished in her wheelchair alone in her room ( a prohibited, unsafe practice which a nurse said she would address) or in front of the tv. She lost ninety percent of her personal ( very nice)” clothing and wore rather sloppy outfits. Same socks more than one day in a row. There was supposedly an activity director but I don’t know why there were no activities (that I saw anyway). I didn’t call often but if I called the nurses I either could not get through get through at all or had to leave a message. I NEVER received a return call, even once. We once could not find her at the hospital they said they sent her to late at night after she had a fall. We called a number of hospitals to no avail. We called the nurse urgently hoping to find her and they never called us back. Through our own detective work we finally found her. On the plus side she was clean though her hair was not always very fresh and she had oral bad breath a lot. She never had any ulcers and they did treat the ones she came in with (resulting from a stay in rehab after a broken hip). They also called promptly if she had a fall and. Injury. Food was good. The facility was clean. However, Staff were often seen lounging with their cell phones out when things were slow and once a young man was semi dozing, head resting on the counter in the dining room for quite a while. . I never saw any staff make eye contact with her when serving her a meal. They’d slap a plate down and keep moving. No @hello, here you go, enjoy your meal”-. No interaction. It was overall rather sad. We removed her and she is now in a small group home where they care for her lovingly and constantly update us. They know her name, her interests, they even say a short prayer with her at bedtime. Mom is very religious, it’s a comfort to her.

Do not use Wellsprings for skilled rehab! My mother came to Wellsprings after a hospital stay with Covid to rebuild her strength using PT and OT. She required assistance to use the bathroom to prevent her from falling. The staff allowed her to fall going to the bathroom by not using a gait belt for safety. The in house doctor didn’t see her for 5 days. My mother wasn’t on oxygen when she went to Wellsprings yet within 4 days she required it. Chest X-rays were ordered yet never taken. I transferred her to another facility on day 8. The new facility took an X-ray next day to find pneumonia as well as her being dehydrated. She was transferred to the ER and died 3 days later. If you have a loved one needing care, look elsewhere. There are a few caring workers at Wellsprings; but, the majority should have a different profession if they do not care for the wellbeing of their patients.

My mom was in this facility after hip surgery during Covid in 2020. She didn’t make it out of the 10 day quarantine period before she re-injured her hip and had to have the same surgery. This was negligence on the part of the Wellspring staff because why was no one there to assist her in bathing. Even if she possibly asked to bathe alone they should’ve been there! She called me from the ER because I was supposed to get her laundry that day. The staff had no answers when I went to the facility and the manager of that floor was extremely rude the next day and didn’t have answers as to what happened. The director of nursing was nice enough to make me not take further actions and tried to convince me they would take better care of my mom when she returned and place her on another floor with a different manager. I would never put anyone else I love in this facility. I constantly took or sent my mom food because theirs looked disgusting. At the last minute I realized she needed rehab and at the time these reviews were good. Do not use this facility for anyone you care about. The staff don’t care and are definitely there to collect a paycheck. I should’ve did this review years ago but it was on my heart today.

My grandmother walked in on her own on Friday for memory care.After being unable to get ahold of her or anyone else to check on her well being all weekend my mother decided to personally check on her. What she found was horrific. She was completely neglected and forgotten about. On Monday morning she was wearing the same clothes she went in with in Friday, her sanitary bits were unchanged, she was severely malnourished and dehydrated. The staff was rude and nasty. They refused to call 911 or have her seen by a doctor. We personally called 911 and had her transported to the ER. It is now Thursday, the doctors don''t think she is going to bounce back from the severe neglect and called in hospice. If you love your family Wellsprings is not the place to send them.
