Sunol Creek is designed, staffed and dedicated to the daily care and medical needs of those with Alzheimer’s and Dementia related illness. Since it’s opening in 2014, Sunol Creek has established itself as the East Bay’s Premier Memory Care community, proudly providing compassionate care for families in Pleasanton, Livermore, Dublin and Fremont.

Sunol Creek was purposefully built for memory care –each neighborhood of 20 residents has its own internal courtyard, circular indoor walking path, ample natural lighting, an activity room, integrated nurse call/delayed egress door systems and various room types.

Levels of Memory Care
Sunol Creek offers two care neighborhoods, each designed to meet the physical, emotional and spiritual needs of our residents. With just 20 residents per neighborhood, we are unique in providing a safe and manageable neighborhood size that balances supervision with ample interaction among residents, staff and activities specialists.
Memory Care – Our “South” Neighborhood offers personalized care for those coping with early to mid-stage dementia or Alzheimer’s.
Enhanced Memory Care – Our “North” Neighborhood offers more intense medical care, complex medication management and is specifically designed to meet the needs of those with significant medical ailments and/or later stage cognitive impairment.
Health Services
The staff at Sunol Creek includes our Medical Director, Licensed Vocational Nurses (LVNs), and Certified Nursing Assistants (CNAs). Our medical staff considers the resident physician’s report, medication list, and preplacement appraisal for more appropriate personal care plans. Initial care plan are drafted by our resident care coordinator and health services director and our care plans are updated 30 days after move in and within every six months or a change in condition. We present care plans to families during family care conferences and welcome their input.
Sunol Creek’s medication management program is run by our Health Services Director. We staff LVNs at every shift, and we are able to accommodate higher needs residents with wound care, catheter care, diabetes, high blood pressure as well as complex medication management.
Activities and our Award Winning SPARK Program
It is easy to identify the limits of Alzheimer’s and dementia, but through our partnership with the Center for Applied Research in Dementia, we prefer to focus on what our residents can still do, and the abilities they bring to our Community. Sunol Creek, via its unique SPARK program, offers residents a variety of Montessori Method inspired activities as well as traditional social activities, outings, and community initiatives.
Meals on Wheels, Kids Against Hunger, and reading to preschool kids are also some of the events that our residents participate in. Recently, we had an Elvis impersonator come to the community for a performance, and the residents loved it. Our furry visitors, including a St. Bernard that has earned the affectionate nickname, “Burt the Flirt”, are always a big hit!
One of the highlights of our community is our restaurant-quality meals. Our On-Site Executive Chef prepares a wide variety of entrees from our general menu but also offers additional selections for residents who may want to have something different.

Sunol Creek features, five apartment styles; private suites, three different configurations of private rooms with shared bathrooms and shared companion rooms, all meeting various resident needs and budgets. There are ample common areas including activity rooms, family-style dining rooms, resident “therapy” kitchens, fire places, a salon, a family meeting room and a private dining room. I can confidently say that I sleep well at night knowing that he is very well cared for.Nurse Heidi is highly skilled in her position and very knowledgeable in all aspects of care, doctors, hospitals, medicare, hospice and follows the doctors prescription orders.The Administrator Jessica has been in the profession for many years and the staff and moral and care that she hires is always top notch. She has worked very hard to create a beautiful community. I can truly say that she is right where she is supposed to be. I hope she continues to share her wisdom and knowledge to other communities as well.The admitting/marketing director Susan held my hand through the entire step of the way as I admitted my husband into their care. She helped me through the process and was always there when I needed her. We also shed a tear as well as this was very hard for me.Last but not least...you must say Hi to Ricky. When my family finally made the choice to transition my dad to a facility, we had Susan, the Marketing Director, out to our home. Well let me tell you, she is good at what she does. We were sold a bill of goods that SCMC was unable to live up to. When my father became agitated and anxious, which is VERY common in dementia patients, the Head Nurse, Heidi's solution was to put him on Ativan. Our doctors refused to prescribe it, as it is terrible for dementia patients. The staff at SCMC's solution? We were asked to hire a one-on-one caregiver to sit with my dad for 9 hours a day, 5 days a week to solve the issue. (Actually, they wanted the sitter there for 12 hours a day, every day, but I refused.) So on top of the ghastly amount we were already paying, we were forced to almost double our fees in order to keep my dad there. And to top it all off, when we finally decided to move him to a smaller facility (best decision we ever made), they sent my dad off with half of his clothes missing, dressed him in someone else's shirt and shoes (that were two full sizes too big), sets of bedding that were not his, and his glasses were nowhere to be found. It was just a slap in the face of what was already a horrible 4 months in their care.One of Susan's big selling points is that SCMC has never asked one of their residents to leave; that they stick it out and work with the family to find the best solution for their loved one's care. This is an absolute farce. Their way of keeping this stat is to make things miserable for the families until they are forced to leave. If you care for your loved one, I absolutely recommend you pass on this overrated excuse for Senior care.

There are few decisions more painful in life than admitting it is time to place your loved one in a facility for their health, safety, and continued love of life, for whatever time that might be. I spent many months searching the internet, reading reviews, and typing my list of questions. I interviewed 12 facilities in the east bay, and as my list dwindled to 3 which I thought my mother would enjoy and I felt secure about having her in their care. I made unannounced visits to those selected facilities to see the day-to-day working of the staff with residents and the levels of engagement offered. When seeking memory care, I found that it was not necessarily a personnel issue that affected my choice but the thoughtfulness of the building itself. The ability for residents to walk freely and not be stopped by metal doors was a critical consideration for me. Many of the facilities had no freedom to any garden area or fresh air. Sunol Creek Memory Care, a small community, was the only place I had found that was a circular hallway surrounding a lovely garden, with Barbeques, tables, umbrellas, and, yes, fresh air and sunshine. Sunol Creek, although small, has a higher level of staff when compared to more expansive facilities. The care, cleaning, maintenance, cooking, nursing, marketing staffs are diligent in earning and maintaining the trust you have placed in them. The Health Services Director, Hiedi, or as most of the families call her, our "beautiful Nurse Hiedi," is one of those rare individuals who never puts anything above the care of our loved ones or, as she calls them, "her family." Her nursing credentials are impressive, her knowledge of Dementia and Alzheimer's behaviors, medications, and professional working relationship many Doctors make her invaluable in the care of the residents and a mentor to nursing and care staff. Hiedi makes all the residents light up with her infectious smile. She even wears glitter-dusted tennis shoes because my mother loves shiny things. I would never intrust my mothers' life to someone I would not trust with my own. Thank you, Hiedi. When it is your time to make a painful decision, consider Sunol Creek Memory Care.
