This is absolutely the best place! My mom has a wonderful apartment, and she loves living there. They have in-house rehabilitation, which she needed after hip replacement. I will recommend this facility to everyone!

Very simply put, I don’t think that I can say enough positive things about the Swan Creek to really articulate how I feel. Our dad was a resident here for about the past year. He was first on the independent living side before moving over to assisted and then eventually skilled care in his last days. The facility is great inside and out. He/we loved the nature-centric setting and really liked walks along the Swan Creek on the new boardwalk. The Swan Creek staff, they are the real hero’s in my mind. From the aides, nurses, and Hospice folks he saw every day (or nearly) all the way to the kitchen staff, facility crew, and administrators that he interacted with less often, they are all truly amazing people - giving, understanding, warm, and kind. Living 2 hours away, even with my sister living close to the facility, this loving staff that I felt made our dad feel like he was part of their larger “family” right there really helped me cope with an already very difficult situation. If the folks at Sawn Creek are reading this, my sister and I would like to thank you for all you did to support us and our dad. We couldn’t have had him in our lives as long as we did without you help and for that we are forever grateful.

Ohio Living Swan Creek in Toledo, OH--I would say that the support that I receive here from the staff is great and that it is a place that does accept Medicaid and they also have a "Care Fund" to draw from so that no one is ever denied care as long as they live. I'm very happy that "A Place for Mom" directed us here. Now my husband is in Memory Care and he is just down the hall from my Independent Living apartment.
