My elderly sister,[name removed], broke her hip in late 2022. [name removed] was released from the Hospital around the first of January, 2023. Nurses from Sacred Heart Home Health were assigned to help her recover at home. The nurses and I quickly found out she was no longer capable to live at home alone. One of the Health Care Nurses, [name removed], agreed and suggested transferring her to a Assisted Care Senior Center, called Oak Gardens Place in Altoona, Wisconsin, to help her recover. To coin a phrase, it was what the doctor ordered. Initially she was upset she could not live at home alone but within the first couple weeks realized she needed help and accepted the care she was being given. Starting with the Director [name removed], I and my sister [name removed] cannot begin to thank the administration, nurses, and staff for the positive assistance she received. I and my sister highly recommend Oak Gardens Place for your senior love ones. They will get the care they deserve.

The staff at Oak Gardens Place was exceptional. They were extremely professional and extremely friendly. Tony, the Community Director, was amazing. You could tell he had a lot of experience. He wanted to find out things about my mom, her likes and stuff, right away. He spent a lot of time with her. He just did a great job. They had an exercise class, and they had a library there. Tony just knew people that like to do the same things as my mom and he's going to set her up with them to do crafts, sewing, and music. It's an excellent place. It's just too expensive for us. Otherwise, that would be the place we would like her to go, but it just didn't work out.

Oak Gardens Place has good transportation access. It's one block from the bus stop and my brother could walk to that. It is our favorite right now. The only thing we don't like about it is the rooms are pretty small and they seem to have a lot of staffing transition. The manager checks in with me every two to three months and we talk and update. They were very courteous and attentive. The rooms were nice, the facility was clean, and they have home cooking there. The meals were great. They had us dine there and the food was good. We liked the staff that we talked to. Though there were some changes in staffing, the changes have been fine. We've already met the current head and we like him. He was my favorite one there. He wasn't the director of the whole center, but he was the director of sales and he was attentive to my brother's needs. He seemed to get what we needed and try to give us more information about those particular details. The facility was new, well kept, clean, and modern. They had nice group rooms and amenities. They had a piano and my brother likes to play the piano. They had a nice bright dining room with lots of windows where the people could look outside and in the summer, they have things going on outside, like little gardening projects and things like that. He would enjoy that a lot. A lot of the food is cooked in the facility. Communication is very good. They follow up pretty often with me, and they're always very courteous. The cleanliness was very good, and the residents can get extra help with that, too. They've been really good about checking in with us.

She's borderline between assisted living and memory care, and I didn't think that would be enough care for her. I did a tour of it, and the staff called me a couple of times. I thought he was very, very good. They offered meals, they had laundry, and they had a patio area.
