Great facility my father spent some time there was a good experience for him and may continue services with them in the near future...I will say probably best to note to name tag your personal belongings....
They should be closed ! The assisted that are living there don''t get the help and treatment they should. The people working there don''t know how to help or treat them. Plus there are not enough workers, 20/25 people living there and 3 workers and 1 RN. thats their Behavioral Unit.

First off let me say I applied to this facility last week apparently men of color like myself(BLACK american) are not on the (to higher) list i walk in after being invited in for interview after appling and sending resume online the receptionist gave me a look like i discusted her but i remained professional interview with staffing coordinator was great nice woman but received a call from the apparently RACIST humane resources lady she then told me because of my 6 year old DUI she wouldnt hire me First off it doesnt matter if u have a DUI in nursing if not a felony n it comes off after 5 years n ive worked at other facilities since n never was an issue but she decided after seeing the ethnicity on the app that she wouldnt hire me n plus the pay was an insult 15 n hour for a LNA with 19 years experience n NEVER is late or calls off and already showed fingerprint clearance card from the state of arizona Wal-Mart pays 15 an hour to start avoid this cheap racist facility step into the 21st century n pay your staff appropriately n stop discrimination i have the right to work n support my family just like anyone else filling formal complaint with state board of nursing and BBB today not one black nursing staff spotted the whole time i was there only one worked in the kitchen im spreading the word on what i experienced at this place currently highered at 2 facilities they both said same thing 7 year old dui charge does not prevent u from working as cna or nurse never has n i said u discriminate against black americans 50% of your staff might identify as hispanic but u made it clear that black americans are not welcome on your nursing staff with the way i was treated thats fine the words out n your pay is terrible anyway shows what you think of your cna staff giving them peanuts i am working making alot more in a facility that is diverse and welcomes ALL qualified applicants like myself aviod this place you can do alot better

I have been familiar with the Lifestream community along with many of my colleagues for years. The first thing that comes to mind is their dedication to advocating for patients, and their ability and willingness to help and support challenging situations families face. The Lifestream administration and staff have consistently demonstrated that they have a strong awareness of community needs and a commitment to meeting those diverse needs. Their care teams have been responsive, available and they have proven than they go above and beyond for anyone who comes through their doors. What I have appreciated most is their dedication to developing collaborative relationships with the healthcare community so that their service delivery and program development is evolving continuously to meet the changing needs of everyone they serve. Lifestream is one of a kind. They come through with genuine care, concern and compassion when you need it most.
Read moreThis by far is one of the most cleanest and friendliest communities that I have ever visited. The staff is very welcoming and helpful. Once you walk in the main doors you get this very welcoming aroma in the air. I would recommend this community for your loved one. Thanks for the great experience!
