If I could give this place a 0 I would. It was dirty, it smelled, the staff were rude a lied about things or differed everything to say we had to speak to someone else. . ( only 3 staff were ok- one nurse the night my father arrived, one PT and one gentleman who gave my Dad meds the day he left) medications weren’t given as directed. My father went there for rehab and we pulled him out of there within 48 hours. After reviewing his paperwork after we left, they had an expected discharge on there for 6/5. That’s a joke, just to suck money from his insurance company. He needed to rebuild his strength after a fall he did not need 3 months there. Call bells went unanswered while 3 plus staff members sat at the desk talking and eating. The took away all dignity from my father and put him in a diaper the minute he got there. My father did not need a diaper , he was fully capable to use a urinal and walk to the bathroom assisted. When they finally removed the diaper and my father asked to go to the bathroom and requested a walker an aide refused to give him a walker- and said come on let’s go, grabbed his arm to assist him to go to the bathroom. My father said he almost fell and had to lean on the other patients bed while walking to the bathroom. In the evening staff left a towel in the main entrance door ( fire safety and patient safety hazard) and several staff members sat at the front door smoking. I have been in healthcare for 30 plus years and it is a disgrace that place and people are there to care for people. If we left my Dad there he would have withered away. There aren’t even chairs in the patients rooms that are safe for them to sit in. When I stated to the social worker they didn’t even have appropriate chairs. She said they can bring a wheelchair in- that’s not the answer, if you consider your self a rehab center then the patients should have what they need to effectively rehab. The list could go on, on the things I saw in the 48 hours my Dad was there. But if you really want your loved one to recover DO NOT SEND them there! Please note this review is for the short term ( rehab) and long term care section, the positive reviews and pictures are likely the independent living section - which has been redone. But please keep in mind if you are in the independent living section - no matter how nice it is , since you are a resident if you end up needing short term or long term care you would likely end up in their short term or long term care. So think twice before going into their independent living too!

Springfield Rehab neglected my mother whose health greatly deteriorated while she was in their care. She''s now in hospice with weeks to live. Had she not been neglected at Springfield, including not being properly fed, I believe she would still be in this position. While at Springfield, she lost 10 pounds and we had to remove her from the facility to take her to an ER where it was found that she was severely dehydrated and malnourished, with oxygen levels at 90, and kidney failure. She did not have these issues when she arrived to Springfield a few days prior. She was fed fried chicken despite not having teeth. The nurse practitioner (among other staff) is either highly incompetent or actively committing healthcare violations. This facility is liable for my mother''s health.

I tried to call to check on a family member I couldn’t reach no nurses station the emergency contact was a answering service, and when somebody did answer the phone they told me they was too busy trying to feed the residents and I explain to them. It was a possible medical emergency they hung up and now they’re not even answering my phone calls. When you call The Director of nursing in case of emergency number was the answering service. They will get back to you within the next 24 hours my family members primary care trying to reach out to them as well.

My experience at Springfield has exceeded my expectations. The staff is wonderful very friendly and extremely helpful. I would definitely recommend Springfield rehab for all your nursing and rehab needs.
