Yes the place is old and could definitely use some upgrading however the staff is very friendly and seem very caring of there patients which obviously should be the main concern

I don’t even want to give this facility 1star. Unfortunately Flagler county does not have many choices for rehab facilities and that’s sad. My family and I had to make a decision quickly to place my mom and dad in a rehab facility with not much time to look around. My mother was left on the toilet for over 10 minutes even though she was pushing the call button. The staffs excuse for not attending to her sooner was that the light wasn’t blinking. The food was horrible, and they switched her medication without reading her medical recorded first!!! She ended up with bed sores as well. After her first 2 nights they started an isolation period where family had to gown and glove up to visit the patient. There was nowhere to put dirty gowns and gloves so we threw them in the rooms trash. My mom said the nurse got mad at her because her trash was full. Uncalled for. Patients have to share a room and a bathroom which I find very unsanitary. It’s an old building that’s dirty and stinky. And smells. They spent 5 days and for my mom I had to call 911 to get her out of there. They didn’t feel it necessary to get the hospital involved in what was going on with her health. The nurse lied to me as well. My dad waited over 3 hours for his meds. We had to find the nursing staff to get his meds!! He was brought in from the hospital with a wound on his toe that was healing with antibiotics. He’s a diabetic as well. They didn’t have all the meds on the list as asked the family if we could bring some from home. The hospital doesn’t even allow that. Ridiculous!! Too many strikes against them. So if your loved one needs to get rehab, think long and hard about where they go. If you have to go out of the county, do so! Do not send them here!! You’ll be sorry like our family!!

My brother was in dire need of long-term care, and I was at my wit''s end. Flagler Health & Rehabilitation was recommended to me. My first contact was with Kristy Trevino of Admissions. She is a lovely, compassionate person who took the time to listen to me. She began leading me through the process. The level of communication was and still is second to none. Shortly thereafter, a tour of the facility was arranged. I met another delightful lady named Dawn who was most informative and helpful as well. Kristy was our tour guide. All of our questions and concerns were readily answered with a warm, personable touch. The staff was accessible and friendly. We settled my brother in two days later in a smooth transition. Since then, the dialogue between my family and the center has been transparent--all replies to our inquiries are responded to in a timely fashion. Big shout out to Momma Bear Kristy Trevino for going above and beyond her call of duty to solve any pop up issues during this process!😘 Placing a loved one in a care center is one of the most difficult decisions to reach, and then implement. But I truly feel Flagler Health & Rehabilitation was an answered prayer during a time of great emotional upheaval brought about by my brother''s abrupt and tragic loss of health and independence. I ask any and all of a spiritual persuasion to pray for my brother and the Earth Angels who care for him and the many souls in the same circumstances. 💜🙏

The admission process for my father was excellent. Both the admissions director and the administrator were involved in the process and even gave me their cell numbers in case I had any questions. The LPN who checked him in was very thorough and patient. I got there at a little after 11 AM and they even arranged to be sure he had lunch. The staff is very kind and understanding. Thank you!!
