My mother in law was here for rehab and and became strong enough to discharge home and had a very good experience . Love having an in-house Nurse practitioner available for good care! Would definitely recommend Creekside.

Creekside Rehab is responsible for my moms death. She was significantly neglected, would ring her button and receive no help for over 4 hours, she was a fall risk and had to go to the restroom with no assistance multiple times. Techs could barely speak English and wouldn’t help when they were on their breaks. Nurses were rude. We were not told that patients had Covid when we checked her in straight from the ICU (in which her room smelled like sour mold, the ac was leaking, and there were multiple water stains in the ceiling and water under the floor) After a few days she was complaining of not feeling well. The day before we were sent the text attached stating creekside had 6 new patients with Covid. My mom ended up in the ER with Covid-19, a bladder infection, and dehydration. The website boasts "newly remodeled" and I asked the nurses about it and they said they had no clue when it was remodeled. The facility is far from the website photos and video. I spoke with the DON and another nursing manager - nothing changed with her care. They did change the ceiling tiles after I left a bad google review which they asked me to remove — but replaced them while my mom was in the room eating and white dusty particles were getting all over me, the room, and my moms food. My mom missed her cancer treatment apt due to having Covid and quickly declined from the Covid. I will be taking legal action. No one should send their loved ones here. My mom had 6 months to live and it was cut to 3 weeks. I have multiple texts and voicemails from my mom about the neglect during her stay.
