Had a loved one here a year ago. Place is really pretty but the care is atrocious. The front desk lady is the most sour and unfriendly person I have ever met. She just seems miserable all the time. Why in the world would a business keep someone like that on board, when they are the first point of contact for everyone who walks through the door? Next, no staff is there seemingly. It''s like a ghost town walking through the halls. Very eerie. Third, my loved one was served food with mold on it. I was horrified. I grabbed the head nurse and showed her. She replied, gross! And got her a new meal. If I hadn''t been there, my loved one who has dementia, would have chomped away at that dirty meal unknowingly, and possibly gotten sick. I was devastated and transferred them immediately out of there. SMH.

I am writing this on behalf of a former patient who is not tech savvy and unable to do a google review. I had her permission to post this review. Her name is Kathy, and she was at another facility on a ventilator. She was unable to be weaned because the facility she was at did not provide that service. She came to our facility with the hope of being weaned off the ventilator, have her trach removed and go home with her sister. She was at our facility for 6 months. With the help of nursing, the respiratory therapists and the pulmonary nurse practitioner she was able to be weaned and went home a month or so ago. She wanted you to know her experience at the Avenue at Broadview Heights was 5 star and feels if the wheels were not put in motion by caring individuals who saw her weaning potential and went above and beyond, she would not be home. She stated she was eternally grateful.

Our mother was brought to The Avenue for rehabilitation after a fall. At the end of her allowed medicare days they said we would have to pay cash until we could find her another place to live.. But then she developed an infection and had to have IV antibiotics. Then she fell again. We were told Medicare would now allow additional days. We were then billed $1200 for those days. She fell 4 times while she was there each time needing a CT scan and ambulance ride to the hospital. The hospital found clots in both her legs and her lungs. She also had bed sores from being left in the bed and wheelchair for too long. I tried to address these issues with a nurse named Sandy (??) and she got defensive and rude. I placed a call to the director to call me back but never got a reply. I also told the front desk person I needed a call back and told her what happened also. Still no response. This is a state-of-the-art facility but don''t be fooled."

Horrible experience, I got hired and no one communicated with me, about my hire stuff. Super rude to me. I will never be going back again. I don’t recommend this facility to ANYONE.
A few shining stars as far as the staff goes, but would never recommend to send anyone to this facility. Very disappointed in the follow up and inadequate care.
