This facility has changed it''s name many times over the last five years, making it hard to understand what is really happening at this facility because you won''t see past reviews. There is no social worker. They are not holding care plan meetings. Facility communication is very poor and your loved one will be cared for by different people most shift as they don''t keep specific nurses on a floor longterm to establish relationships with the people they serve. Steer clear of this facility and utilize other SNFs until there are significant changes.

DO NOT LET YOUR FAMILY “RECEIVE CARE” HERE. Not only do they not take the time to learn about their patients, but they also don’t even provide them with basic needs. Refusing to take them to toilet, not showering the patients, unsure of why their patient is there and not aware of their current health conditions. Oh and don’t try calling them because they will tell you they don’t have your family member there WHEN THEY DO. I have never experienced a place this careless.
