Worst place ever!!!! I came in everyday, three times a day because I was so worried for my moms safety. I came in one day to find blood on her sheets and went to find her at the nurses station. I asked why there is blood on my moms sheets and the her nurse said she had fell and had a skin tear on her arm when I looked I noticed her wrist was broken so badly it was deformed looking. They never apologized, no phone call that she fell, not one word from them. They could care less. If you care at all how your loved ones are treated this facility is not for them. I finally got her in middlesboro nursing home, which is the best by far. I plan on having these to old nurses in particular that treated my mom so horribly, to be held accountable for what they did by the state of Tn. I wouldn’t even give them one star but I did to make this public.

My mother in law spent 2 weeks and 3 days at Tri State Health and Rehabilitation facility and I would highly recommend that if anyone is having to place a friend or loved one in a health and rehab place, then this is the place you would want them to be. The care she received was excellent, and in my mother in law’s words, they treated her like a queen. She said the food was great and all the therapist, nurses, and staff were so sweet, kind, friendly and jolly. They made her stay there a very pleasant one, and also helped her children and family be able to sleep at night knowing she was being so well taken care of. The place was clean and nice smelling soon as you walked in the front doors and all the rooms and residents that I saw were clean and well taken care of. Thank you Tri State Health and Rehab for taking such great care of my mother in law. Myself, her children, and all her family thank you. God Bless you all and all your residents🙏🏻
