I was a patient 9/22--25/2020 for physical rehab. I called 911 and got myself out. I was having back spasms and pain, requests for adjustments to meds were ignored. Call bell was not working. Only way get help was to yell for help. I was reprimanded for doing that even after I explained why. Staff would come in but not introduce themselves. I was in to relearn to walk. First day with physical therapy, the therapist asked the longest distance I could walk. I was told we would be doing that that day. At that point I could only stand with a walker and two person assistance for 15 seconds. The final draw was Friday 9/25. At 1:45pm I was placed in a wheelchair for physical therapy. Due to incontinence issues I wet my adult diaper at 2pm. I asked to be changed. Request was denied. From 2pm to 6:45pm I had urinated in the diaper 3 more times. Each time I asked to be changed. Each time I was denied. I was in a urine soaked diaper for approximately 4 hours. I had been dealing with severe back spasms since arrival zt the facility. I asked my nurse for a change of medication. She stated she would contact my doctor. One hour later I had not gotten a response on the issue. As a redult I developed chest pains and was having a hard breathing. Request to be seen at the hospital was denied by CNA. I ended up calling EMS myself with my cell phone. The staff was unaware of this. When EMS arrived the nurse in charge refused to let me be taken to ER. EMS asked if I was being abused. I denied being abused but stated I was being neglected and explained the situation to them. After that I was transported by them to Meritus Medical ER due to my pulse oximeter readings being low..I would not recommend this facility to my worst enemy
