Staff was attentive; the rehab was excellent, facility was very well maintained, and we were very happy with the overall care.

My Mom was at LASA for the past 8 years and three months to be exact until she passed away September 16, 2022. This was her home and she was well cared for by all the people that surrounded her during her stay. Laura and her team (Aida, Yen, Josie) are amazing at making sure activities went on before, and during the pandemic to ease the boredom of the residents. In addition they also took on additional tasks during the Covid isolation time frame. This is a never ending thank you to Helen, Cora, Rosa, Fabiola, Yadira, Eric who helped take care of my Mom, especially with the collaboration with Heartland Hospice care. Mom went peaceful at her home called Los Altos Sub-Acute & Rehabilitation Center. I miss my Mom and will miss the wonderful staff of LASA. George, thank you so much for caring not only my Mom but all of the residents at LASA and the support of the staff to enhance the care and services they provide. Keep up the good service.

For five days with many many requests, no nurse, no doctor, no one from this facility update the status of the patient. Four days after, we put in a request to transfer to a different facility and no one did anything at the facility. The last day, the manger was supposed to arrange for transport to get the patient home, she left work without telling anybody. I did not recommend anybody to sent their loved one there.

My grandma was sent here. Such a horrible place! She got a horrible bed sore because she was not being tended too, staff didn’t do anything to prevent this from happening like they should have done! She is now in the hospital with an infection that this place tried to say was pneumonia! She didn’t even have that, it was the bed sore! There was also an incident where we heard they gave her insulin (she’s not diabetic) , they told us they did and wen we asked to confirm again, they decided to say they never gave her that. Bad , bad , bad , please look for another place to take your loved one!

You expect skilled nursing and get complaints from the staff on how overwhelmed they''re by having too many patients. The shouldn''t even be fed to dogs. The fee per day was requested to me was $414.60 a day. The therapy department is a joke. They spend less than a thirty minutes with you and they bill you for a hours work. The ombudsman is your only protection from terrible treatment.
