One of the worst Healthcare facilities I have ever been in. My friend was placed there with 1 leg amputated and the other leg was broken. She was placed there for therapy. First day, the bed was broken and the wheel chair they put her in was shredding its wheels. No working remote for the T.V. She is non weight bearing on the broken leg. She put her light on to go to the bathroom and no one came for 30 minutes and she had an accident. That happened twice. So now she goes to the bathroom on her own No one even comes to her room to ask how she''s going to the bathroom. She''s been there for weeks and has had therapy twice. Really? One day the nurse brought her meds in and she told the nurse she didn''t recognize the pill. The nurse said she would check. It was not her pill. She asked for toilet paper because she was using paper towels. Staff took her clothes after her accident and they were not returned as of yet. She''s asked at least 5 times and no one can find them. I would never put a loved one in this facility. I just feel that this is not the way its supposed to be run.

Not good. Mother in law was placed at Integrity. Like most health care facilities you have good and bad staff. We had some issues with one employee in particular. Very rude. Our loved ones room was not cleaned properly. Ants everywhere and when you walked in her room, your feet stuck to the floo...

Not good. Mother in law was placed at Integrity. Like most health care facilities you have good and bad staff. We had some issues with one employee in particular. Very rude. Our loved ones room was not cleaned properly. Ants everywhere and when you walked in her room, your feet stuck to the floor. Instead of putting depends on my Mother in law, they told her to just use the bathroom where she was. We couldn''t get her out of their fast enough. She is now in a great facility that she loves. I would like to add that the HCU administrator was notified and she did respond to these issues.

My mother was sent for rehab from a break. 7 days she stayed in same clothes and was putting herself in and out of the bed. We done a care plan but none of the CNA will follow and housekeeping is awful. This place was recommended because of rehab. I made several complaints, night staff was rude and...

My mother was sent for rehab from a break. 7 days she stayed in same clothes and was putting herself in and out of the bed. We done a care plan but none of the CNA will follow and housekeeping is awful. This place was recommended because of rehab. I made several complaints, night staff was rude and would be unprofessional and take it out on my mom. I came by late to check most were sitting at the front desk. Definitely understaffed and untrained, and will not follow head nurses orders. Upper management tried to fix it but staff just didn''t care.
