I wasn''t sure I wanted to do a review until I received an email saying they would gladly help me with upcoming care I may need. Hawthorne is the LAST place I would EVER go. The 3 stars are for the staff & the PT & OT people. They are outstanding. Only 2 people I''d fire if I was in charge. Everyone else is super amazing & the best. The food, if you can call it that, is terrible. I liked breakfast because I ate cheerios!! Don''t take your loved ones here!! It''s dirty. They sweep, but I NEVER saw disinfectant floor cleaning & I personally know body fluids hit the floor. I went there for PT due to knee surgery & went home with FIVE infections. Wound up back in the hospital a week later. I was actually becoming delirious from the infections. Be careful if you choose Hawthorne!! I was there in August 2023.

Hawthorne is a joke! They are rude, unhelpful the admissions department is ridiculous. One minute they answer the next week they don''t answer or call back. Dont send your loved ones. here.
My Dad was in the facility for wound care for bed sores on his but and recovering from surgery on his wounds. The care he got was terrible zero rating
Absolutely superb! The CARE from the staff is superb and with a smile! The facility and the grounds make this a lovely place to be!!

In this small town there are a handful of rehabilitation centers. My dad has been in 2 of them. From the outside, we were always told Hawthorne was the best for their rehab piece. His first time staying at Hawthorne was 2020 when he had a partial hip replacement. It was in the midst of the COVID pandemic starting. Their staff always seemed knowledgable and helpful, weekends were like a ghost town with less help but we were able to visit more when not working. Fast forward to this year, he had a few falls and was sent to rehab to build his strength. First few days in the facility and he fell again. He got admitted on a Friday and over the weekend it seemed that no one was working as it took anywhere from 30-45 minutes for his help light to be addressed. We complained that Monday and the admissions lady helped us write up a grievance form. His medication didn''t come over from the hospital correctly and the two nurses on duty (I believe one was training)- were very nice and helpful and called the doctor for us and got the issues fixed. After the fall, we asked for his room to be moved and they obliged. He had his moments where he pushed his button and couldn''t get help and he would call us and we would call the facility (after hours is the worst as NO ONE answers) and we would ask them to go address his needs and they would. He spent almost 2 months in the facility, about a month before discharge we had a care plan meeting where they advised us of his progress and they gave us an estimated discharge date. They also asked us as a family if we had any questions, concerns, etc. I made mention that the walker he was using at home (4 wheels, brakes, seat) was too small for him and he needed a walker for taller people. I was told they would look into it for us. A few weeks later about a week before discharge I received a phone call asking about the Home health agency we would like to use for his OT/PT/Speech, I advised of a company we had worked with before and had great results, I even gave the lady their phone number. At that time I also asked about the special walker we had requested in the meeting and the woman I spoke with acted like surprised like this wasn''t something discussed at the care plan meeting. Fast forward again to the day he got discharged, I arrive to find a brand new plain Jane silver walker (which we have one at home already) and not the one requested. I immediately call the company on the invoice so that we aren''t charged for a walker we didn''t need. I bring my dad home and later that night I receive a phone call from not-the home health we requested. I advised we didn''t need their services as we requested a different agency. Since that day (7/20) I have called Hawthorne countless times a day and left at least 6 messages for someone in social services to call me back. Still no response. So now my dad has been home for almost a week and no nurse has been out and he hasn''t received any OT/PT. If Hawthorne cared about their patients as much as they say they do, they would''ve rectified this situation last week when I left the first message. It is sad that one employee can tarnish such a good experience so quickly.
