My mother was moved there after a hip replacement. The facility is understaffed, dilapidated. The sidewalk needs to be repaired to avoid trip hazards and resident falls. Med Tech and Caregivers are not properly trained an the turnover is ridiculous. Theft of resident property as well as visitor propery has occurred. The new director will not return calls from family members and cancels meetings after a letter was sent to family members to reach out to her to hear our concerns. There are many vacant rooms as well. This place needs help.

We are working with memory care, the director of nursing is very aware of everything happening with the residents and in tune with what is going on. I would recommend to others looking.
There were a few issues with keeping up with daily things like laundry, and showers. Their communication between staff members was not that good and there were not on the same page. Overall they tried to do their best but I would only recommend this community is staffing improved.
Flagstone is a beautiful community,awesome staff and the residents are outstanding.
