Update: I wanted to post an update on this negative review. Tammy the social worker is a complete nightmare. She advised my extended family that I threatened to sue her which is not true. I just threatened that my attorney would report the nursing home to the states ombudsman after she kept giving me the run around. This woman is insufferable. She lied straight through her teeth regarding their in house doctor not approving my Grandmother to leave in which case is hilarious because my Grandmother told me she never even saw a doctor while she was there for the 6 weeks. Additionally she LIED about my Grandmother being charged for the full stay and providing her with a bizarre invoice when I picked her up. Scaring the elderly with FAKE HEFTY BILLS is unethical and pathetic and that woman should be fired. I reported her nonetheless for her scare tactics alone. Despicable. We are 9 months out after my Grandmother signed herself out and she is happy and healthy! Don’t put your loved one in that dirty rotten nursing home if you can help it! I generally do not feel compelled to leave reviews, but this situation certainly requires one. My Grandmother chose this nursing facility after falling and breaking her hip early Feb 2020 because she knew one of the aides and knew the aide would make sure she received great care. The nurses and aides in fact provided excellent care and were very compassionate and polite as well as the PT''s. My hang-up is with the social worker named Tammy. My Grandmother had expressed on multiple occasions she would like to be discharged/ leave, which to my knowledge, the words are synonymous. It fell on deaf ears. Tammy continually told my Grandmother that she needed additional rehab and their internal doctor needed to approve her leave, which the doctor apparently was not going to. My Grandmother got to the point in rehab she was fully walking unassisted, dressing herself, making her bed, and using the restroom. At this point, I contacted Tammy again and advised her I talked to my Grandmothers doctors office that said they saw no problem with her coming home and they would provide home health PT. This fell on deaf ears and she brushed me off, and advised me that they needed to do a home evaluation to clear her to go home, but could not provide me with a date this would occur. At this point I was sick of the run around. I received another call from my Grandmother advising me they were in fact not letting her go home because Medicare wouldn''t pay if she left against medical advice. Knowing this was NOT true I contacted my attorney. I needed his advice on how to get her out considering Ohio has nursing homes on lock-down not allowing visitors due to Covid-19 and this social worker was using it as her advantage because I was unable to have these conversations in person with her or my grandmother. At my wits end, I contacted Tammy, demanded that my Grandmother be released because she was extremely unhappy not being able to have no visitors, letting her know that Medicare not paying was a lie because I contacted them, advising her that we do NOT need a doctors order for my Grandmother to leave AMA, and that I wanted her discharged the SAME day! Tammy was extremely rude and condescending and wouldn''t stop talking for me to get a word in edge wise. I then advised her I consulted with my attorney and that if this did not happen he would contact our states Ombudsman office on my behalf. Shortly thereafter my Grandma was released and I will never go back to that place again! note: Medicare pays 1-20 days in full for skilled care. 21-100 days is 80% and if you have a supplemental insurance they most likely pick up the rest. Medicare has the highest reimbursement for skilled care so make sure that nursing homes do not recommend "more care" than whats needed to try to keep your loved one there because they just want to milk Medicare! Be sure to do your homework, ask around, and consult with attorneys or else the Nursing home will keep your loved one until the money runs out!

The first day my mom was there she wanted out. She said it was horrible. But due to her horrible Obama care insurance she couldn''t go anywhere else. The staff is extremely rude. You call up there and random people answer and they have no clue what they are doing. They transfer you to another person or to a voice mail. They will 100% lie to you and tell you what you want to hear to get you off the phone but they wont actually call back and follow up with any calls after they lie and say they will. My mom ended up dying here and not a single thing added up as to what happened. I think it was very shady how they all acted. My mom would have gotten better care in a prison! If you care about your loved one dont take them here! I probably wont ever get the answers I was wanting and that is wrong! They sure gladly took all the money that was paid to them! Shady place that is listed under different names. The so called new director is no different than the other one. I think they mark down your number if you call up there more than once because their tone changes quickly if you say you called before. Im sorry my mother was forced to live her last days here because of our broken health system!
