I am shocked and appalled that some people gave this facility such a poor rating I was in that facility for two and a half years I was treated with the utmostof respect for my entire state the therapy department under the direction of Sunita gave me the best physical therapy and the best occupational therapy that anyone could ask for the receptionist that greets people Miss Angel she is so extremely professional the way that she greets visitors yes patients,answers the phone she is just fantastic as a matter of the nurses that I had and the gnas they treated me with the utmost of respect and looked out for any problems that I had Miss Baker the director of activities she was utterly fantastic I went through a few administrators while I was there but the last administrator that I had Miss Emily Logan on a scale of a 1 to 10 she is about a 15 I met with her everyday and we had a wonderful understanding when I knew I was going to be leaving she was more than willing to do anything she could to assist me with my transition I will always be grateful to a facility that helped me when I needed it the most, when I first got there I was unable to stand on my own two feet today I am able to stand and take steps. To all of the staff and the Administrator of Autumn lake at Bridge Park I give my most heartfelt thanks and appreciation for everything that you did to help me again thank you. MAY GOD HIMSELF BLESS YOU ALL!!! P.S. AND ALSO I WOULD LIKE TO THANK THE DIRECTOR OF NURSING AND THE ASSISTANT DIRECTOR OF NURSING MISS TONI FOR EVERYTHING THAT THEY DID TO HELP ME AS WELL! If I could give this location zero stars I would. My mother was sent here because it was one of the only places avaliable that could provide Trach care. Day one her room was invested with ants and roaches, the dietary restrictions/instructions from the hospital were blatantly ignored. Now these are all obstacles that can be overcome with just a fraction of attention from any staff member, but that did not happen. Day three my mother ripped her Trach out and we weren''t informed until hours later. We wanted to call 911 and have her taken back to a hospital to be checked over, because she hadn''t been able to pass any of the Trach trials prior to her leaving the hospital. We were informed by several staff members that we couldn''t do that and they could deny the EMTs at the door. They said they''d call an ambulance when they thought she was in danger. (Things that I''ve learned after the fact from this is that facilities like this are judged based on 911 calls, so don''t hesitate to call an ambulance if you feel your loved one is in danger. They cannot turn EMTs away) Eventually they call 911and transport my mother to a hospital, where we learned that she had become septic because of an infection. Now that she was at the hospital we to get her stuff from the facility and we wererefused. So all of my moms belongings are somewhere at this facility and they won''t give any of it back. I''m blessed to have my mother still with us, stuff is stuff we can always replace things. I cannot express enough that you do not want to send your loved one here. My advice when looking for rehab centers is to do your research! Look at reviews, visit the facilities if you''re able.

Great staff as far as I can see !!...although they need to answer the phone at the nurses station and pay more attention to the patients especially because of the facility being respiratory !

I was diagnosed with Covid in August 2021. After nearly ten months in the hospital, I was sent to Autumn Lakes for rehab. Trach in, unable to walk, and relying on being cared for by others. From the first day here, I have been treated with respect, and given top notch care. The techs, nurses, PT/OT staff and even administrators and insurance specialists have been amazing. I have read all the reviews made by others and I’m sorry that others didn’t feel this same love I do. I would highly recommend this facility. I fought like heck to not come here because I had read all the reviews and allowed myself to be guided by what others had written. I have A long road to recovery, but, I know I’m in the right place.
