I agree with a couple other reviews- end of 2018 they assisted with elderly abuse and exploitation. Moved a senior citizen about 120 miles away from their home so they can isolate and use undue influence on elderly. Gave elderly new medication which patient did not have any disease or disorder for and the side effects made elderly have blurry vision and hallucinations. Then employee and relative had elderly sign legal documents that could not be understood by elderly-fellow employees were used as witnesses. Threatened to take elderly home and be kicked out in the streets if elderly did not sign documents. Elderly finally stopped taking all medications and eating because they did not trust the caregivers at facility. POA finally got changed to correct person, but Azalea Trails did everything they could to delay moving patient- it took 4 weeks to get elderly moved out to another facility.

My mother was here for a few months and it was the worst mistake I’ve ever made. The food that they serve is terrible. There’s no care in the menu. They even lost my mom! She had a doctors appointment and a cna was to accompany her. Well I received a phone call from the nurse asking when was I bringing my mom back. I told her that I was at work and I never took my mom out. I left work horrified and my mom has dementia. I called the police and they located my mom at the university of Chicago!!!! She was scared and horrified. The director and staff kept calling me apologizing and that it wouldn’t happen again. Be warned, the staff is constantly on the cell phones and they don’t bathe residents regularly. A lot of the nurses have attitudes. It’s truly a negative place .

My Mother In Law Has Fallen Several Times Because Of There Neglect And Lack Of Nursing Skills She Fell And Broke Her Hip. Had To Have It Replaced. The First Day Back From The Hospital She Fell On The Hip That Was Replaced. We Are Trying To Move Her To A Better Facility. The Staff Is Doing Every Thing They Can To Block This Move. My Mother In Law Has Dementia And They Are Using That To Their Advantage. The Staff Is On A Power Trip And The Devil Is Allowing This To Happen.
