3/25/2024- My Mom has been in this rehab facility for one month as of today. When she first got there all I could think of was getting her out. I made a call to the corporate office and the very next day I started seeing changes. I was at the facility daily so I saw the changes being made to the staff as well as maintenence in the rooms. They have a new Administrator. The nurses and CNA''s are now being kind and attentive to the patients. Physical therapy is a dynamic team who have worked hard together to help Mom get back on her feet and she will soon be coming home. The NEW Social Services Director has been instrumental in getting Mom items she needs both while she is there and going home.While I did not originally care for this facility I can say without a doubt that I would feel good about taking her back to this facility if needed.

My father went 2 WEEKS without bathing!!When he finally came home,i removed his socks to find that his legs was covered in grime/dirt!His feet was shedding in the socks too,discusting.If you have family that you love,STAY AWAY FROM THIS PIS PLACE!

I worked there through agency for many years off and on.I recently worked there for about 2 months.People are being abused and neglected .Sometimes the people who are needing help getting fed ...don''t get fed.The director KAY has a theft ring going on amongst the cnas.and if you speak up you get fired.They let a trachea patient lay in soil on south wing flushed properly.Jerica Greene is guttervillle and so is Stacy Blue..they are bullies.Another thing they do is swap medications amongst patients.They also kept 1500 of my pay.Ive hired an attorney and reached out to state and the labor board for my wages.. the trachea family complained but there''s nowhere else to put his mom. Another patient needed emergency surgery after leaving there because his peg tube was never tended to..They allow patients to get alcohol delivered to a point they are falling down drunk and needing ems.This place is a license in jeopardy kind of place. Kay jerica and were all hired within the last year.The place has really turned into hell for families and patients..And Josh the administration guy will promise families the world just for a check.

This facility is the worst facility I have ever been to. My dad is currently there for rehab. He has been neglecting since day one. He has been there since 12/26 and to this day has not been offered or given the supplies ( wash clothes/ towels/ soap) to bath. They never gave him a toothbrush/ toothpaste. He never has sheets on his bed when I arrive. The room is dirty, shoes stick to the floor when you walk on it, there is dirt and small debris in and around the bed, there is an odor of urine the minute you step into the hallway leading you to the rooms. Every time I go I spend the first 5 minutes making his bed, disinfecting the side table, gathering the full garbage bags and taking out the trash. The nurses and physicians have neglected him. There is no healthcare at all. This place is committing medicare fraud big time. The call light stays on for one to two hours to the point that I have to call corporate and get them to contact one of the nurse supervisors that lives near and she has to drive up to the facility to have the staff provide care due to no one answers the phone after 5pm. This has happened more then once. Dylan in corporate definitely does a good job of telling me what I want to hear and then doing absolutely nothing to have the situation changed. Due to the holidays I have not been able to report them however tomorrow I will be calling the ombudsman, Medicare , Advent Health ( they transferred my dad here) to make a formal complaint. I will also be filing a grievance. My dad has requested to talk to a dr since day one and was told not until next week on Tuesday after the holidays. We all know Dr are on call 24/7. He is a chronic pain patient and had to beg for pain medication. I had to get involved for this to happen. The nurses/cna are rude, unprofessional and sloppy. They lie repeatedly. I am a nurse of 19 years and would be embarrassed to provide the level of care this place does. These employees belong working in a prison or not in healthcare at all. My dad being in this facility has caused me extreme anxiety to the point I am unable to sleep at night. I am doing everything in my power to get him out of there.

My great grandmother was there for 2 years and by far the worst place ever. She was very malnourished when she was transferred and when asked about it they said that she refused to eat; which is very untrue. She also had various lesions and bruises on her body. This place should be shut down ASAP!
