Do not send anyone here!! This place needs to be SHUT DOWN IMMEDIATELY!! They wouldn''t give my 77 year old unhealthy grandma her breathing treatments or her medications that she needed. When she got them back as she was leaving she was missing quite a few. Unsure if workers took them or gave them to other patients! She had to do her physical therapy in her room with 2 other people she said all she did was walk around in circles for her therapy for 5-10 minutes a day and couldn''t leave the room to do therapy because she wasn''t "allowed out of the room due to being at the hospital prior." She said all she did was walk around in circles for 5 minutes.. Her roommate needed help and she didn''t have a buzzer on her bed. So my grandma had to keep pressing it for her. THEY DID NOT COME AFTER HOURS OF PRESSING IT! My grandma had to call 911! I WOULD NOT SEND ANYONE HERE!!!!!!!!!

POSITIVES: Most of the staff are friendly and welcoming. The place is clean. The entryway usually has a staff at the front desk during visiting hours. The front door is alarmed and locked to prevent residents from wandering. (Note when no one is at the desk you cannot leave the facility through the front door. It is locked and alarmed) NEGATIVES: even with family visiting every single day, the follow-through of staff is poor. Ex. My diabetic mother was receiving mostly starch and very little protein in her meals. The dietitian and three staff said they would get this adjusted and it took 3 days to accomplish. For 4 days we were promised some cushioning gauze behind the ear of my mother because the oxygen tube was causing a sore. It never happened, even though I saw a nurse speak to the wound care nurse to obtain this. My mother developed a pressure sore from lack of attention/care. It was discovered by family members when they walked in on her being changed. We were told it would be cared for by the wound care nurse. For 3 days we could not get this taken care of even with family members advocating for this daily and the aide stating they had reported this to the nurse several times. Additionally, if her diaper was soiled, it irritated the wound even more. The end result was when she went to the hospital the pressure sore was infected and was so bad it required surgical treatment. INCONSISTENT CARE: Multiple therapy sessions were missed because my mother''s diaper was soiled and we could not get a staff person to change her so therapy could take place. My mother is obese and because of the challenge of working with her they allowed her to remain in bed and continue to decline in strength and health. She only dressed if family were there to help her. SAFETY CONCERNS: 1. my mother''s oxygen was left off of her by staff after they cleaned her up 4 separate times in 3 days. 2. Frequently staff left her call button out of her reach. 3. When the oxygen concentrator stopped working, they brought in a replacement unit and found it had a broken plug. 4. Some of the outlets are so worn that plugs fall out. This happened to the television but it also happened to the oxygen concentrator. 6. Lack of repositioning for non-ambulatory patients. 7. Neglected skin care . SUMMARY: This may be a good place for ambulatory patients who needs some therapy and minimal care. This this is an extremely dangerous setting for non-ambulatory patients.
