Aging by Design seeks to provide direction and support to caregivers
of aging loved ones. We know coping as a caregiver requires specific tools which help all parties involved
to adjust to the loss of control associated with aging. We are here to help you maintain your balance by
providing step by step guidance and a trusted resource for obtaining knowledge. Our promise is to listen
objectively; and refer you to outstanding services which best serve your needs.
After a thorough assessment to examine your loved one’s environment
Aging by Design identifies problems, eligibility for assistance and the need for services. We develop a
customized care plan that gives you the insight on how to manage your loved one and their future.

- References to appropriate professionals within all aspects of geriatric healthcare
- Coordination of evaluations and other medical appointments
- Liaison activities for families at a distance
- Consumer education, advocacy and care management

When to contact Aging by Design
- You or someone you know is a caregiver who is overwhelmed or stressed
- Support and resources are needed to maintain your loved one’s quality of life and independence
- There are multiple diagnoses/difficult situations that complicate decision making and care coordination
- Family members disagree about the next step; need information and objectivity
- Someone is struggling with balancing safety and independence
- Family is out of town and /or limited social support
- You find yourself or your family needing consistent, unbiased, accurate communications
- Existing caregivers require supervision and ongoing training

Jessica Gill, MPH, CMC For the last 10 years Jessica has focused on caring for the elderly,
educating family members, and working on various elder care topics. Jessica has worked for the
Centers for Disease Control, the Arthritis Foundation National office, Sunrise Senior Living and the
Southeastern Geriatric Healthcare Group. Her niche lies in providing resources to help make the
quality of life better for all those affected by the limitations of aging.
